For New Patients

Bring any forms you need the Dr. to fill out (school forms, medical leave, disability, back to work clearance, etc.)

Bring any documents that maybe helpful for treatment (Physician referrals, psychological testing results, school reports, etc.)

If you would like for us to verify your insurance benefits before the appointment, please call our office at 949-282-0027 and provide us with your insurance information.

Patient copays, co-insurance and deductible payments are due at time of visit. we except cash, check, visa, master card, and discover.
